What LEMR replaces

Everything Your Agency Runs on, in One Place

Stop stitching together an EHR, a billing service, a scheduler, and a payroll tool. LEMR was designed as a single connected system so data is entered once and flows everywhere it's needed.

Clinical Documentation

OASIS-E1, CMS-485 plans of care, discipline evaluations, visit notes, wound assessments, and aide care plans — all in a patient chart that is the single system of record.

Billing & Revenue

PDGM grouping, Medicare 837I and NC Medicaid 837P claims, a pre-submission scrubber, eligibility checks, denial management, and LUPA early-warning.

CMS Compliance Calendar

OASIS transmission, the NOA clock, PECOS revalidation, QRP, HHVBP, PEPPER, HHCAHPS, and beneficiary notices — tracked so deadlines don't slip.

Scheduling & Visits

Schedules that pre-populate from a signed 485, GPS clock-in with exception flags, visit-cadence tracking, and 21st Century Cures Act EVV export.

Payroll & Workforce

Timesheets with differentials, W-2 and 1099 payroll, ACH and ADP export, credential and license tracking, onboarding, and an agency policy library.

Mobile for the Field

Native iPhone and Android apps with Face ID sign-in, voice-to-text documentation, photo capture, offline support, and a HIPAA background-lock.

Clinical documentation

Charting That Keeps Clinicians Inside Their Scope

The patient chart is the system of record. Every instrument — OASIS-E1, the CMS-485, discipline evaluations, visit notes, wound assessments — lives in one timeline, and the platform enforces who can sign what.

  • Complete OASIS-E1 — all RFA timepoints, consistency-edit checks, and derived BIMS, PHQ, and functional scores.
  • Scope of practice enforced — a PTA cannot sign a PT evaluation; an LPN cannot complete an RN-only OASIS.
  • Voice-to-text documentation and structured wound photo capture, right from the field.
  • Goals & outcomes authored once, referenced by the 485 and visit notes, and rolled up into agency attainment.
Billing & revenue

Get Claims Out Clean — And Catch Revenue Before It's Lost

LEMR ships with the real CY2026 CMS PDGM data, generates compliant X12 claim files, and surfaces the problems that quietly cost agencies money.

  • Accurate PDGM grouping from a 43,000-code ICD-10 table and all 432 case-mix weights.
  • Pre-submission scrubber for 837P and 837I, with 835, 999, and 277CA response handling.
  • LUPA watch projects every open period before the threshold is missed — not after.
  • Denial worklist with CARC reference, aging buckets, and appeal tracking.
Scheduling & operations

From a Signed Care Plan to a Verified Visit

Finalize a 485 and the schedule fills itself for the certification period. Clinicians clock in from the patient's home; LEMR captures the location and exception flags automatically.

  • Schedule auto-population spreads each ordered visit frequency across the episode.
  • GPS visit verification with clock-in/out, mileage, and late/early exception flags.
  • EVV export builds the six-element Cures Act record from every completed visit.
  • Visit-cadence tracking shows ordered-vs-delivered per discipline, in real time.
Who LEMR is for

One Platform, Both Sides of Your Business

Agencies rarely run a single line of service. LEMR adapts the patient chart, billing, and workflows to each one.

Home Health

Medicare-Certified Agencies

The full Medicare workflow — 60-day certification episodes, OASIS-E1, the CMS-485, per-discipline visit documentation, PDGM and 837I billing, and the complete CMS quality and reporting calendar.

Home Care

Private-Duty & Medicaid Agencies

Personal care and CAP service lines with demographics-based enrollment, comprehensive assessments, ADL documentation, NC Medicaid 837P billing, prior-authorization tracking, and EVV.

Getting started

From First Call to First Visit — Guided the Whole Way

We provision a private, isolated workspace for your agency and walk your team through it. Most agencies are documenting live within a few weeks.

Discovery Demo

We learn your service lines, payers, and disciplines, then show LEMR mapped to your real workflow.

2

Workspace Setup

We stand up your own isolated tenant — branding, locations, staff roles, payers, and policies configured with you.

3

Team Training

Role-based training for clinicians, schedulers, and billing, plus a built-in classroom and policy library.

4

Go Live

You start documenting and billing with our support team on call for your first cycles.

Security & privacy

Protected Health Information, Treated That Way

LEMR is engineered around HIPAA from the data model up — not bolted on. Every agency is fully isolated, every key action is logged, and a Business Associate Agreement is included with every subscription.

Encryption

Data encrypted in transit and at rest on Google Cloud infrastructure.

Tenant Isolation

Three independent layers keep one agency's data invisible to every other.

Audit Trail

Every key clinical and administrative action is recorded and reviewable.

Device Safeguards

Auto-logout, mobile background-lock, and biometric sign-in by default.

Common questions

Answers Before You Commit

Can we move our existing patients and data into LEMR?

Yes. During onboarding our team works with you to bring patient demographics, active care plans, staff records, and payer setup into your workspace so you go live with your real census, not a blank slate.

Does LEMR work without an internet connection in the field?

The mobile apps include offline support so clinicians can keep working in homes with poor connectivity; data syncs when a connection returns. An agency-wide downtime package can also generate offline PDF backups of schedules and charts.

Is LEMR available on iPhone and Android?

LEMR runs on the web and as native iPhone and Android apps from the same account. The mobile apps add Face ID / Touch ID sign-in, voice-to-text documentation, photo capture, and a HIPAA background-lock. The iOS and Android apps are rolling out through the App Store and Google Play.

Do you sign a Business Associate Agreement?

Yes. A BAA is included with every LEMR subscription, and our underlying infrastructure providers operate under their own BAAs. See the Security page for details.

Which states and payers do you support?

LEMR supports Medicare home health nationwide and NC Medicaid billing today, with additional state Medicaid and payroll-tax support added as agencies onboard. Tell us your state and payers on a demo call and we'll confirm fit.
